Effects of semaglutide on kidney disease in type 2 diabetes: a randomized placebo-controlled trial

2 October 2026 · Nature Medicine

Nature Medicine published a randomized placebo-controlled trial examining how semaglutide may protect the kidneys in people with type 2 diabetes and chronic kidney disease. The study tested once-weekly subcutaneous semaglutide 1 mg against placebo over 52 weeks in 106 participants, including 25 women and 81 men. The research team included Katherine R. Tuttle, Petter Bjornstad, Matthias Kretzler, and colleagues across institutions including Providence, the University of Washington, the University of Michigan, Lausanne University Hospital, University Health Network in Toronto, and Novo Nordisk.

The trial was designed less as a simple outcomes study and more as a mechanistic investigation. Researchers used multiparametric kidney MRI, kidney biopsies in a subset of 33 participants, single-nucleus transcriptomics in 22, and spatial transcriptomics in 13, comparing before-and-after treatment measurements. The co-primary MRI outcomes, including oxygenation, global perfusion, and tissue inflammation, were not significantly changed by semaglutide versus placebo. But secondary findings suggested semaglutide reduced renal artery resistive index and stabilized apparent diffusion coefficient, signals consistent with reduced vascular resistance and prevention of fibrosis progression.

The study matters because diabetic kidney disease remains one of the most consequential complications of type 2 diabetes, and GLP-1 receptor agonists have shown kidney benefits without the mechanisms being fully clear. The transcriptomic results pointed toward effects on glomerular endothelial cells, with spatial analyses suggesting fewer immune cells near those endothelial cells. In plain terms, the findings suggest semaglutide’s kidney protection may involve healthier kidney blood-vessel function, less fibrotic progression, and improved molecular programs in key kidney cells. The trial is registered as NCT04865770.
